Our Mission

Philly CEAL is dedicated to addressing chronic disease disparities in underserved communities in Philadelphia, particularly focused on improving cardiovascular disease (CVD) management among Philadelphia residents.

The CEAL Approach

It Matters

Heart disease is a leading cause of death in Philadelphia, especially among racial and ethnic minorities. This project seeks to address not only medical needs but also social determinants of health, such as food insecurity and housing instability, which heavily impact health.

Community Engagement

Philly CEAL is a partnership between the University of Pennsylvania's School of Nursing, the City of Philadelphia’s Office of Community Empowerment and Opportunity (CEO), and multiple community-based organizations working together to promote health equity.

Innovative Approach

Philly CEAL combines the DECIDE self-management program, which teaches people problem-solving and decision-making skills for managing their health, with the continuous support of CHWs. This holistic approach focuses on both the medical and social needs of participants.

Impact

Our program aims to reduce hospitalizations, improve quality of life, and empower community members to take control of their health. It is also expected to provide valuable insights for other cities and communities looking to implement similar health interventions.

Community Health Workers

Community Health Workers provide essential support to community members in managing chronic diseases, like hypertension and diabetes, and help connect people with resources for food, housing, and healthcare, which are critical for improving health outcomes.
